Summary of Position: The Intake Supervisor is responsible for the supervision of the Intake Department. Coordination of homecare services ordered by physicians, scheduling appropriate disciplines to meet the home care needs of the patients.
Essential Functions:
- Communicate and collaborate with Liaisons and/or referral sources as needed to transition patients to home care timely
- Responsible for reviewing patient referral documents to make sure that they are accurate and comply with regulatory requirements. Contact referral sources for any missing or additional information, including orders/documentation as needed
- Oversees and assists with entry of referral information in the agency’s electronic medical record system accurately
- Creates, evaluates and implements intake processes and procedures as needed
- Reports referral activity on a daily basis. Track referrals that are received and not admitted to identify any possible trends
Education and Experience:
- High school diploma required; Associates degree preferred
- Experience using electronic medical records and Microsoft Suite Software
- 2 years home care intake/scheduling experience

Experience:
- supervisory: 1 year (Preferred)
- intake/scheduling: 2 years (Preferred)
- clinical: 2 years (Preferred)
