Job Title: Case Manager/Supportive Services Specialist

Department: Business Development Reports To: Program Director FLSA Status: Exempt

General Purpose of the Job: Reports to the Program Director/VTC Operations Director of the Herbert

J. Hoelter Vocational Training Center (HJH/VTC), the Case Manager will provide a range of client- centered services that connects NCIA’s HJH/VTC program participants with community and employment services to ensure timely and coordinated access to supportive services to remove barriers that affect training completion. The Case Manager will work to ensure that the client’s goals meet all required supportive services. This role is responsible for monitoring participant progress and documents performance as specified in the Individual Employment Development Plan and as required by NCIA’s data collection policy. The Case Manager serves as the client’s career advocate and liaison between the client and staff in the implementation and follow up of programmatic and barrier removal issues.

Essential Duties and Responsibilities

A. Essential Functions

1) Provides case management services to the HJH/VTC program participants to assure that the client goals and outcomes lead to increased self-advocacy, self-sufficiency, community integration and gainful employment.

2) Provides information, referrals, and assistance with linkage to supportive services.

3) Function as a liaison with other service providers and helps maintain collaborative community and employer partnerships.

4) Monthly reviews (or as needed), with NCIA’s program participants to identify the client’s career values and barrier removal services needed.

5) Assists with the determination of the client’s career goals and outcomes throughout their cohort training time.

6) With input from the Work Force Development and Instruction staff creates the individual employment development plans for each client’s identified goals.

7) Works with NCIA’s leadership team or their designated point of contact to conducts in-service training for required staff on the proper implementation and delivery of the Individual Employment Development Plan.

8) Enters all relative and pertinent case management documentation into NSS, Works, and ETO.

9) Provides daily, weekly, monthly, quarterly, and final case management performance reporting based on grant and proposal requirements.

10) Provides daily, weekly, monthly, and quarterly attendance for student meetings based on grant and proposal requirements.

11) Participates in all staff meetings to provide real time updates on barrier removal progress, internal and external barriers to training completion and job placement.

12) Works to achieve and obtain required percentage or higher meeting success rate for NCIA Vocational training and Work Force Development Institute program participants.

13) Works to obtain required percentage or higher success rate for all barrier removal services provided to NCIA program participants, measured by performance management reporting within the Electronic Data Management System (NSS) and NCIA’s leadership observations or their designated point of contact.

14) Works to maintain required percentage or higher performance satisfaction rate per participant.

15) As an Independent Contractor agrees to adhere to NCIA’s Core Values and Code of Conduct Policy as outlined by the Company.

16) Ensures all information entered the HJH/VTC electronic data management system.

17) Complies with federal, state, and company policies, procedures, and regulations.

B. Secondary Duties:

1) Completes special projects as needed.

2) Performs other related duties as requested.

Supervisory Responsibilities: None.

Education and/or Experience:

Ø AA Degree or 5 years’ experience human services.

Ø Bachelors accredited college degree

Language Skills:

Ø Ability to read and understand information and ideas presented in writing.

Ø Ability to communicate information and ideas in writing so others will understand.

Reasoning Ability:

Ø Ability to solve practical problems.

Ø Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Certificates, Licenses, Registrations: None.

Other Skills and Abilities:

Ø Possesses collaborative work style, combined with ability and desire to work in a team-based environment.

Ø Expertise in Microsoft Office applications.

Ø Excellent customer service and interpersonal skills.

Ø Attention to detail.

Physical Demands:

Ø Communication and auditory ability required.

Ø Ability to lift to 20 lbs. occasionally.

Ø Ability to stand, sit, walk, bend, climb, and reach.

Ø Ability to see (vision) details at close range (within a few feet of the observer).

Work Environment:

Ø Primarily indoor office environment.
